they actually don't require much custom work at all. The only thing you'd need to do is possibly work the mettle a little to where they actually connect to the body. Other than that, there's nothing special in the shape of them other than the trailing edge if you get the "factory" Mercedes versions. And if you do that, the best bet is to buy the stock fenders as well so you can get the indention as well. Then you'd cut and weld what you need into position on your fenders to meet up with the front section of the doors. And body work the rest. That's how my fender vents were done. So that way the natural curves that meet the vents are still in tact:
Other than that, there's aftermarket versions which are not contoured like the facory ones and are easier to pretty much "bolt on". The brown car above in your post has the after market versions.

You can buy the insert at a benz dealer. I THINK they are $67.50 a side but that might be for the W163 hood inserts. It's been a while since I priced it out.
The fake cheesy stick on SL vents look like shit in person. The real SL vents look much nicer. Here they are on a GS:
My problem with SL vents on lexus's is that some people already say the GS looks too much like a benz already. Now i'm not one to care what people say (at all) but there is some validity in that statement. You could make some VERY clean fenders like the ones below for ~$20
If you need help I will gladly do a DIY.

Man I am glad I did not get those vents. That guy added the chrome parts to it (which I would have stayed away from) the grill and hoodbadge don't help it either.
I did see a white GS a few weeks ago with the chrome version of that grill and in the second to bottom left slot he had the "LEXUS" badge and I thought it really took any benzness away from it. I gave him the thumbs up but he gave me a shitty look :(
I think those are my favorite to date. Perhaps just a notch below the magnifique.

Just make a smaller vent on the lower cladding instead.
Easier to remove and play with off the car and install right back when done.

This is like three sets of wheels ago, but...
These are all metal with a metal mesh.
Fenders were pulled and lips trimmed off before paint.
Not exactly "SL vents," but I wanted something a bit unique.

yep this is nice...
If I did something to fenders it would have to be as clean and simple as that^^
